Children ages 12 -17
Following the recent announcement by the National Department of Health (NDoH) that the vaccination of children aged 12 to 17 years of age against COVID-19 have opened, we are inviting all our students who qualify to be vaccinated on the 25 and 26 November 2021.
SCHOOL COVID-19 VACCINATIONS AT SACRED HEART COLLEGE
Following the recent announcement by the National Department of Health (NDoH) that the vaccination of children aged 12 to 17 years of age against COVID-19 have opened, we are inviting all our students who qualify to be vaccinated on the 25 and 26 November 2021.
We invite you to extend this vaccination initiative to other friends and family members to take this opportunity to get vaccinated on any of these two day.
Dis-Chem will be providing the vaccinations and they take guidance from the NDoH protocols, which currently provides for a single dose of the Pfizer vaccine for children aged 12 to 17 years of age.
Please note the following guidelines regarding this vaccination initiative must be followed:
- Eligible and participating (and friends/ family members, if applicable) must have pre registered on https://vaccine.enroll.health.gov.za/
- Please send the COVID-19 VACCINATION FORM (copy attached to this letter), copy of ID or birth certificate and a copy of medical aid card (if applicable) with your child by the 19 November 2021 . This will allow us to give the exact numbers to Dis-Chem so that no vaccines are wasted or run out.
- In line with the Children‘s Act, children aged 12 to 17 may access medical treatment without their parent’s consent, however we as a school will ask for parents, or guardians to sign the attached consent form as well.
